A 45-year-old patient named Sandra called us six months ago. He reported that one of his teeth occasionally hurt when eating and then he got a treatment of cosmetic dental crowns.

The pain was very sharp but lacked the pain normally associated with a hole, fracture, or abscess.

Sandra even looked closely at the tooth in the mirror and found no visible fracture or receding gum line. The dentist suspected that he might have broken teeth syndrome.

This is a condition that occurs when one or more fine cracks form in the tooth. Fractured tooth syndrome is not the same as what people commonly call a broken tooth. In fractured tooth syndrome, the cracks are almost always too small for a person to see.

Even dentists have to use special instruments to detect them in most cases.

Cracked tooth syndrome can have several causes. Some develop slowly and others are the result of severe trauma to the mouth. Chewing repeatedly over a period of time can cause very fine cracks called stress fractures, leading to cracked teeth syndrome.

Bruxism (or grinding your teeth at night) can also cause teeth to break.

A closer examination of the tooth with our intraoral camera confirmed that it did indeed suffer from cracked tooth syndrome.

There was a small but very deep crack inside the tooth that was dangerously close to breaking the nerve chamber. If the syndrome had gone unnoticed for just 3-6 weeks, the division would have made its way to the nerve and we would have had to have a root canal.

Instead, and to Sandra’s great relief, we were able to replace the cusp of the tooth and replace it with a crown. Last week Sandra emailed us to thank us and let us know that the pain was completely gone from her tooth.

Metal crowns

Metallic crowns (gold or silver) make them a poor choice for the aesthetic area of ​​the mouth. However, metal crowns work well in the posterior area where aesthetics are not as important.

The underlying metal substructure can give PFMs a less realistic appearance.  Another drawback may be that, in my case, a thin metal edge of the restoration may be visible along the gum line.
